We're All In This Together

At Hueman Kind, support is by no means an easy feat, but through cooperation and community empowerment; we believe we can facilitate progress in this area. We are always striving to make a difference, and invite you to learn more and lend your support.

SUPPORTIVE SERVICES

Scheduling

Our Personal Care Liaisons will manage all the scheduling needs for our members. This service includes:

  • Medical Appointments

  • Dental Appointments

  • Mental Health Appointments

  • Transportation to Appointments

  • Etc.

Community Services

The Community Services program screens Members to determine their eligibility for public resources; such as: SNAP benefits, fuel assistance, and healthcare benefits, among others. A Personal Care Liaison will meet with you in your home or virtually to assess your eligibility for each program.

Family Caregiver Support

Through this program, caregivers can receive or be linked to the following services:

  • One-on-one Peer Support

  • In-home assessment

  • Virtual assessments

  • In-home services

  • Nutrition services

  • Transportation services

  • Support groups

  • Financial counseling

  • Legal referrals

 

Information & Referrals

To get information about the full range of our services, call our Information & Referral Department. You will be asked a series of questions; based on your answers, our Personal Care Liaisons will either refer you to a program at Hueman Kind or to another community organization that can meet your needs.
We encourage you to call and speak with our Personal Care Liaisons. There are so many different programs offered by Hueman Kind and other community organizations that our Liaisons are knowledgeable of, and they will act as your guide.

Money Management

The Money Management Program was developed to assist Members who have difficulty writing checks, balancing their check books and managing their money. Some of these Members may be homebound, visually impaired, confused and without family nearby to help.

The service helps Members who might be at risk of losing their independence due to their inability to pay basic rent, food and utility bills on time. The purpose of the Money Management Program is to promote independent living for Members with limitations; who are at risk because of barriers to manage their financial affairs.

The Money Management Program Services Offered: 

A trained, supervised Personal Care Liaison is carefully matched one-on-one with a Member needing assistance. The Personal Care Liaison meets virtually or goes to the person’s home at least monthly to help keep track of income and expenses.

Two levels of assistance are available:  bill payer and representative payee.

  • Bill Payer

This service assists Members by developing a budget, writing checks and balancing their checkbook. Members make all their own decisions and sign their checks.

  • Representative Payee

This service helps Members who are not able to manage their own finances. Hueman Kind applies to the Social Security Administration (SSA), which includes a Member’s agreement form and Physician’s statement validating the client’s need. Once approved by SSA, a rep payee account is opened in the Member’s name and the Hueman Kind writes and signs checks from this account to pay the client’s bills. This is a fee for service program.

Eligibility Requirements

  • Must meet income and asset guidelines

  • Must have a checking account

Members in Need May Be:

  • Homebound, visually impaired, disabled or confused

  • Without family nearby

  • At risk of losing their independence due to their inability to pay bills on time
